GENERAL NOTICE OF DISSOLUTION OF BROOKE PSYCHOTHERAPY, INC.
Notice is hereby given that Brooke Psychotherapy, Inc., whose last principal office was located at 975 SE Sandy Blvd Ste 160, Portland, OR 97214, was dissolved effective on December 31, 2023. Pursuant to ORS 60.644, all persons having a claim against Brooke Psychotherapy, Inc. are required to present the claim, including the name of claimant(s), address, telephone number, and description of claim(s) alleged, to Karlaina Brooke, at 1140 SW 11th Ave. Ste 500, Portland, OR 97205. Except as provided in ORS 60.644(4), a claim is barred unless a proceeding to enforce the claim is commenced within five (5) years after the date of publication of this notice.
